Ability Urge
- Black/White Version: If chosen Pokémon controlled by its owner has Air Lock, Anticipation, Cloud Nine, Download, Drizzle, Drought, Filter, Forewarn, Frisk, Intimidate, Mold Breaker, Pressure, Sand Stream, Slow Start, Snow Warning, Teravolt, Trace, Turboblaze, or Unnerve, that Ability triggers. Has no effect if that Pokémon isn’t in battle or is rotated out. Cost 3.
Absorb Bulb
- Black/White Version: Whenever holder is damaged by a Water-type attack by another Pokémon, if holder’s Special Attack stat stage is less than 6 and holder’s Special Attack can be changed with this item’s effect, holder’s Special Attack rises by 1 stage and this item is consumed.
Adamant Orb
- Diamond/Pearl Version: If holder’s current species is Dialga, power of Dragon- and Steel-type attacks by holder is multiplied by 1.2.
- Black/White Version: If holder is a Dialga, the power of Dragon- and Steel-type attacks by the holder is multiplied by 4915/4096.
Aguav Berry
- Ruby/Sapphire Version: At the end of each turn, if holder has half its maximum HP or less, holder gains 1/8 of its maximum HP (but not less than 1 HP) and this item is consumed, then, if its nature is Lax, Naive, Naughty, or Rash, it becomes confused if it wasn’t already. (Not prevented by Safeguard or Substitute.) (No effect if used from the menu screen; can’t be used from the Bag.)
- Diamond/Pearl Version: At the end of a Pokémon’s attack segment and each turn, after an attack is used (before fainting if any, but not Pursuit as a target is switching out), and whenever holder or another Pokémon enters the battle, if holder has half its maximum HP or less, holder gains 1/4 of its maximum HP (but not less than 1 HP) and this item is consumed. Air Balloon
- Black/White Version: Holder is immune to damaging Ground-type attacks, and holder is unaffected by Spikes and Toxic Spikes, and holder can switch and run despite Arena Trap, even if the holder doesn’t have Levitate. (Overridden by Gravity, Ingrain, and Smack Down.) Whenever a damaging attack against the holder by another Pokémon is successful, this item is consumed (the item "pops") even if the holder has zero HP. (A message reveals this item on the holder the next time effects from it are applied after the holder enters the battle.)
Amulet Coin
- Gold/Silver Version: Doubles money earned upon winning a trainer battle if a Pokémon (both in battle and not in battle) controlled by the player is holding this item. Doesn’t affect money because of Pay Day. (Not cumulative with multiple instances of this item.)
- Ruby/Sapphire Version: If holder participated in winning a Trainer battle, the player earns twice more money than usual. (This effect is not cumulative.)
- Diamond/Pearl Version: Even if holder has Klutz or is under the effect of Embargo, if holder participated in winning a Trainer battle, the player earns twice more money than usual. (This effect is not cumulative with multiple instances of Luck Incense, this item, or both.)

- Red/Blue Version: The chosen Pokémon controlled by its owner is cured of poison.

Apicot Berry
- Ruby/Sapphire Version: At the end of each turn, if holder has 1/4 of its maximum HP or less and Special Defense stat stage can be raised, raises holder’s Special Defense by 1 stage and this item is consumed.
- Diamond/Pearl Version: At the end of a Pokémon’s attack segment and each turn, after an attack is used (before fainting if any, but not Pursuit as a target is switching out), and whenever holder or another Pokémon enters the battle, if holder has 1/4 of its maximum HP or less, and holder’s Special Defense can be raised, raises holder’s Special Defense by 1 stage and this item is consumed.

Aspear Berry
- Ruby/Sapphire Version: At the end of each turn and each Pokémon’s attack segment and as soon as the holder enters the battle (but not as the battle begins), if holder is frozen, holder is defrosted and this item is consumed. If used from the Bag: The chosen Pokémon is defrosted.
- Diamond/Pearl Version: At the end of a Pokémon’s attack segment and each turn, after an attack is used (before fainting if any, but not Pursuit as a target is switching out), and whenever holder or another Pokémon enters the battle, if holder is frozen, it thaws out and this item is consumed. If used from the Bag: The chosen Pokémon (in battle or not in battle) controlled by its owner thaws out.

Berry
- Gold/Silver Version: At the end of each turn, if the holder’s HP times 2 is less than its maximum HP, holder gains 10 HP and this item is consumed. If used from the Pack: The chosen Pokémon controlled by its owner gains 10 HP.
Berry Juice
- Gold/Silver Version: At the end of each turn, if the holder’s HP times 2 is less than its maximum HP, holder gains 20 HP and this item is consumed. If used from the Pack: The chosen Pokémon controlled by its owner gains 20 HP. (This item is not considered a Berry.)
- Ruby/Sapphire Version: At the end of each turn, if holder has half its maximum HP or less, holder gains 20 HP and this item is consumed. If used from the Bag: The chosen Pokémon gains 20 HP. (This item is not considered a Berry.)

Berserk Gene
- Gold/Silver Version: As soon as a new turn begins (before commands are chosen), if holder is holding this item, its Attack rises by 2 stages and then, unless it is already confused, the holder becomes confused (Safeguard and Substitute don’t affect this), and receives a confusion count of 256 in Pokémon Gold Version, Pokémon Silver Version, and Pokémon Crystal Version. The item is then consumed.
Big Root
- Diamond/Pearl Version: The amount of HP the holder gains because of the effect of Aqua Ring, Ingrain, and Leech Seed is multiplied by 1.3. The amount of HP the holder gains after it uses Absorb, Drain Punch, Dream Eater, Giga Drain, Leech Life, and Mega Drain is multiplied by 1.3. (Affects Liquid Ooze.)
- Black/White Version: The amount of HP the holder gains because of Aqua Ring, Ingrain, and Leech Seed is multiplied by 1.3, rounded half-down. Binding Band
- Black/White Version: Whenever the effect of a multi-turn attack by the holder begins, the effect causes its target to lose 1/8 instead of 1/16 of its maximum HP (but not less than 1 HP) at the end of each turn. (Holder need not be holding this item as the target loses HP this way.)
Bitter Berry
- Gold/Silver Version: Whenever holder becomes confused, or if it’s confused at the end of each turn, it is cured of confusion and this item is consumed. If used from the Pack: The chosen Pokémon controlled by its owner is cured of confusion.
Black Belt
- Gold/Silver Version: Damage of Fighting-type attacks by the holder is multiplied by 1.1 (see Damage calculation).
- Ruby/Sapphire Version: Holder’s Attack for Fighting-type attacks by the holder is multiplied by 1.1.
- Diamond/Pearl Version: Power of Fighting-type attacks by the holder is multiplied by 1.2.
- Black/White Version: Power of Fighting-type attacks by the holder is multiplied by 4915/4096.
Black Flute
- Ruby/Sapphire Version: If used, halves the rate of wild Pokémon battles. (Not consumed after use.)

BrightPowder
- Gold/Silver Version: Reduces the accuracy of attacks with flag "g" by the opposing Pokémon against the holder by 20.
- Ruby/Sapphire Version: Accuracy of attacks by other Pokémon against holder is multiplied by 0.9.
- Diamond/Pearl Version: Accuracy of attacks against the holder is multiplied by 9/10.
- Black/White Version: Accuracy of attacks against the holder is multiplied by 0.9.
Bug Gem
- Black/White Version: As an attack by the holder other than Fire Pledge, Grass Pledge, or Water Pledge begins, if it would deal damage against at least one of its targets and has the same type as that named by this item, that attack’s power is multiplied by 1.5 and this item is consumed before damage calculation. (Counts all hits of a multi-hit attack. Attacks used on different turns are considered separate attacks. The appropriate type item will also trigger if Present would try to recover HP to the target, even if it can’t do so. Affects Selfdestruct and Explosion, even though the holder faints from the attack. For Future Sight and Doom Desire, the effect triggers only at the time the attack is returned at the end of the effect, only if the holder is in battle (even if the holder is rotated out), and only if the holder began the effect by using the attack. For Bide, effect triggers only on the last attack segment of the effect; for two-turn attacks, only on the second attack segment of the effect.)

(Using Trick or Knock Off resets the affected move for its target. Using Trick resets the affected move for its user only if it held no item or a different item from the item gained. If the holder gains this item after using Trick or Thief/Covet and didn’t hold that item just before it was gained this way, it doesn’t become the first move used this way. If this item is activated this way, the item held by the holder is consumed at the beginning of holder’s attack segment (even if it has Klutz or is under the effect of Embargo) unless holder strikes last this turn, and holder’s held item can’t be removed from it, either by the holder or by other Pokémon, and Embargo will fail against the holder, until the end of holder’s attack segment or the end of the turn, whichever comes first. (Affected by Gluttony. No effect if consumed with Bug Bite, Fling, or Pluck. If holder uses Pursuit as a Pokémon is about to switch, it skips its attack segment that turn.) (See timing.)
